PCOS or PMOS, What the changing language means for menopause & perimenopause

There has been growing discussion in medical and research circles about whether the term PCOS (Polycystic Ovary Syndrome) should be updated to better reflect what it actually is, a whole-body metabolic and hormonal condition, not just an ovarian one.

Some researchers and clinicians have started using terms such as PMOS (Polycystic Metabolic/Ovarian Syndrome) or similar variations to highlight the metabolic side of the condition. While this is not yet an official NHS name change, it does reflect an important shift in understanding.

So why does this matter for perimenopause and menopause?

Women with PCOS often enter perimenopause with:

  • Higher baseline insulin resistance
  • More disrupted hormone patterns
  • Greater risk of weight changes and fatigue
  • Ongoing androgen imbalance (like higher testosterone)

As oestrogen naturally declines in perimenopause, some PCOS symptoms may improve, but others like weight, energy dips, and mood changes can feel more pronounced. This is why recognising PCOS as a long-term metabolic condition is becoming increasingly important.

What the NHS is doing

The NHS still uses the term PCOS, but guidance is evolving around lifestyle-first management, including:

  • Weight and insulin management support
  • Hormonal symptom management
  • Fertility advice where needed
  • Mental health and wellbeing support

There is also more awareness now that PCOS doesn’t end at menopause, it simply changes shape.
